Question : I notice that when I am not smiling, it looks like I am frowning. People actually comment that I seem angry.  I don’t know if this involves just my mouth or both my mouth and eyes. Would a consultation help me identify whether I need a cheek lift, an eyelift or both?

Question : I am a 45-year-old woman from Framingham who has been getting regular lip filler injections for the past 3 years. I am getting really tired of this and was wondering if there is a more permanent procedure for lip enhancement?

Answer : I stopped doing permanent lip augmentation about 7 years ago, when I became convinced that hyaluronic acid gives a more natural and more customizable solution.  The permanent solutions often look to “fish mouthed” and can extrude.  I would be happy to see you for an opinion.

Question: I am a 30-year-old woman from Worcester, MA. I have very thin lips. What would be the best option for me if I want fuller lips, but want them to look as natural as possible?

Answer: We could easily see you for a consultation and treatment the same day.  I routinely use hyaluronic acid fillers for lip augmentation, Juvederm, Restylane, Perlane and Belotero.  I have gone away from using permanent implants because the fillers can be more easily personalized. Question :
I'd like to enhance the look of my lips but don’t want a full-on surgical procedure. What are some non-surgical options for a lip enhancement?

Answer :
I like the hyaluronic acid fillers for this. The treatments are less expensive than surgery and can be customized much better than a surgical implant. Here is a link to some photos:

Question:
What filler is good for improving the area under the eyes or the tear troughs?

Answer:
Let me start by saying this is a very “touchy” (pardon the pun) place to inject.  Bruising is common.  The injection must be done very carefully and deep just above the bone.  The skin is very thin so fillers can show through.  If it gets into the orbit or blood vessels of the orbit, it can cause blindness.  That said, a diluted hyaluronic acid filler like Restylane, Juverderm or Belotero can be placed very carefully.  A better solution is a lower lid blepharoplasty with fat transposition.  In this operation the protruding fat pads of the lower eyelid are moved into the tear trough for a permanent improvement.

Question :
I’ve always had very flat cheeks and have envied those with nice high cheekbones. Would implants be a good way to improve my flat cheeks? Are there any other options like fillers?

Answer:
A deep volumizing filler like Radiesse works well for this, and many of our patients choose this option.  However, a silicone plastic implant gives you a permanent augmentation.  Especially in your case, where you have always had a relative skeletal deficiency (ie flatness) implants provide a more logical approach.  There are many different sizes and shapes.  Here is a link to our photo galleries for fillers:

Question
What is a Botox brow lift and how effective is it?

Answer:
This is when a botulinum toxin is used in selected areas to try to elevate the brows.  It can be very effective or moderately effective, depending on your expectations.  A lot depends on how heavy your brows are to begin with, asymmetry of muscle action in your face and the specific does that you need.  We start on the lower end of the scale so as not to elevate the brow too much.  I add more if you want more once we see the result.  It lasts about 3-4 months.  Most of our patents are very pleased with the results.  It is not a substitute for an endoscopic browlift or open browlift.

Question:
Are there any fillers that provide permanent or longer lasting results and what can they be used for?

Answer:
I use a lot of Radiesse.  It lasts 10-14 months in most cases.  The hyaluroinc acids do not seem to last as long.  Sculptra can last longer, but my patients do not seem to want to bear the extra expense.  I use fillers for facial volume increase and for filling lines.  I also use them for lip augmentation (in that case hyaluronic acids).

Question:
I am a 37-year-old female and would like to try Botox injections for my crow's feet and my smile lines. Just curious as to what if any risks are there? And also, typically how long do the effects of the injections last?

Answer:
The most common risk of botulinum toxin (Botox, Dysport or Xeomin) is headache, about 5%.  In the crow’s foot area on rare occasion you can get some drooping of the eyelid, but I have never seen it.  I do not use botulinum toxin around the mouth for smile lines.  Those are better treated with a filler.

Question:
What brand of injectable filler best eliminates deep wrinkles from the sun?

Answer:
It depends on the depth and location of the wrinkles. Usually the hyaluronic acid fillers are Restylane, Perlane and Juvederm work well for this. Perlane is usually my preferred product, lasts longer than Restylane and you get more in the syringe than with Juvederm.
