How to look and feel your best on your big day
Cosmetic procedures before that “big event” are very common. While many people envision major surgical procedures to get ready for these events, there is also much you can do that is more subtle and less expensive. Whether as simple as a botulinum toxin injection, facial filler or starting a good skin care program, there is much you can do. It may be the time to start that program of deep cleansing facials and skin peels that you never got around to booking. You just need to plan ahead. Other than a professional make-up application or skin peel there is not much we can do that will consistently make much difference if you come in for a consultation the week before the event.
Weddings have always been a time when a woman wants to look her best. A bride will want to remember her marriage as a magical moment but the stress of preparing for the big day may leave her face looking less than radiant. A floor-length white gown will also draw the eye to the head and shoulders.
There is no need to cake on a thick layer of foundation for wedding photos when your skin looks fresh and healthy. For beaming brides, skin resurfacing will even out skin tone, reduce wrinkles, and shrink scars. Obagi® skin care will also give you more even pigmentation without surgery. The V-Beam® pulsed dye laser has no real down time, but you might have some redness, swelling or slight bruising for a few days, so plan ahead. You will need to keep on ice on your face for a couple of days. It is an office treatment with tolerable discomfort.
Facial resurfacing and tightening is the process of removing fine lines, acne scars, and wrinkles with the use of Portrait® plasma skin resurfacing, Lumenis® fractionated carbon dioxide laser or Pellevé® radiofrequency treatments. Resurfacing is very popular and the best way to permanently fix facial damage. As you might expect, deeper wrinkle removal requires deeper treatments and, hence, some downtime. Portrait® plasma resurfacing generally requires 7-8 days of downtown and for the carbon dioxide laser 7-10 days. There is no down time for Pellevé®.
Our professional aesthetician can put you on a program of facials and skin peels and consult with you about make up for the big day. She is a professional make-up artist and can do wonders. She can also do Pellevé® treatments to tighten those fine lines around your eyes. The Pellevé® Wrinkle Reduction System feels like a warm massage. We recommend starting with 3-4 treatments one month apart and then consider twice yearly or yearly touch up procedures. It softens wrinkles on the face (especially around the eyes) by slowly heating the deep layers of the skin with a warming device powered by advanced radiowave technology. The heat helps create new collagen, plumps up wrinkles and firms, tightens and brightens skin. There is no downtime and no discomfort when performed by a trained professional (busy patients can go back to work). No drugs or anesthetics are needed. No needles, no downtime, no pain. Facial fillers are very popular. They are injected into skin to smooth wrinkles and to replace volume lost during the process of aging. Restylane®, Perlane®, Radiesse® Juvéderm®, and Belotero® are used to fill in facial crevices and add fullness to areas of the face that have lost their youthful volume. The effects of these injectables last anywhere from 6 to 14 months. We use topical and lnjected local anesthesia to make you as comfortable as possible. Sometimes you may have some bruising for 7- 10 days, so plan accordingly.
Botulinum toxins are called neuromodulators. They are naturally occurring substances harvested from the bacteria Clostridia Botulinum and are used to reduce muscle activity in the area of injection. They all have similar actions and similar molecular structure. Xeomin® is said to be less likely to be susceptible to immunity because it has been cleared of proteins on the molecule not essential to its function in the face. The effects of all three products last about 3-4 months.
The products can be injected in the glabella (the area between the brows) to lighten a scowl, the central forehead for wrinkles and to elevate the outer brow, the crow’s feet, below the corners of the mouth and wrinkles of the upper lip. A new technique called micro injection is also catching on for more subtle results and even to treat Rosacea. There is really minimal discomfort. We use a very tiny needle.
While any of the cosmetic surgical procedures we do can be considered in preparing for a special event, some are done more commonly than others…
The first place someone focuses on when they look at your face is your eyes. Hence, blepharoplasty is often the first cosmetic procedure for patients as they approach or enter middle age. The median age for all patients coming in to see us about their eyes is 43. If you look tired, you will appear older. A number of factors can contribute to an older look around your eyes, excess skin of the upper eyelid (dermatochalasis), low brows (brow ptosis) , fat pushing out the skin of the lower eyelids to give the appearance of “bags” (pseudo fat herniation), and loss of cheek fullness below the eye. Careful and judicious removal or rearrangement of skin and fat around the eyes can open them up. Replacement of fat volume in the cheek below the eye can give a more full, youthful appearance, and addition of cheek volume can restore youthful fullness to our face. Like other plastic surgery, swelling is normal. There should not be much pain. Upper eyelids can be done in the office under local anesthesia. You can expect 7-10 days off of work depending on the extent of the surgery. During this time, patients must not wear contacts, go to a pool or hot tub, smoke or take medications with aspirin or other preparations that can cause bleeding.
Rhinoplasty blends art and structural architecture to restructure your nose. This is a complex procedure requiring a skilled surgeon who strives to balance the nuances of shape and function. It is Dr. Bentkover’s subspecialty. Rhinoplasty is the most common surgical procedure we do for young brides before a wedding, but you really need to plan ahead. Swelling and bruising occur around the eyes and nose. A patient may usually return to work in 7-10 days. While most of the swelling is gone in 4-6 months, It can take up to 18 months to really see the final results. Your nose will usually look reasonably good in person in 3-4 months but may still appear swollen in photos for 6 months.
A face lift or mini face lift can refresh your face. These procedures can restore your jawline, tighten your neck or raise your cheeks. As you age, your facial volume shifts downward, and you loose facial volume. Procedures to restore volume more permanently than a facial filler like a variety of implants or fat transfer can restore volume. Recovery time varies depending on the type of face lift performed, but planning 10 days off from work would be a conservative plan. If all you have is a mini lift of just the cheeks and jawline, it could be less.

I just want to refresh my look for an event without having too much work. What are some procedures you recommend with minimal downtown and maximum results?
I would recommend you start by meeting with our professional aesthetician. She could start you on a skin care program that includes some facials, superficial peels, and skin treatment programs like Obagi Nu-DERM® or Obagi C-Rx®. You should try to start this 3-6 months before the event. You may be a candidate for Pellevé®, a series of painless treatments to tighten the skin around your eyes. You receive one treatment a month for 4 months with no down time. Botulinum toxin injections and facial fillers have no downtime but occasional bruising. Unfortunately the words “minimal downtime” and “maximum results” often do not go together. You may or may not be a candidate for a mini lift or eyelid surgery. However, if you need a full face lift, there is not substitute. The experience with the so-called thread lifts of a few years ago demonstrated this pretty well.
I have had plastic surgery before and have been very pleased with my results however my appearance was noticeably different. Is there a cosmetic procedure that will give me surgical results with a new look that could have potentially been achieved from nonsurgical methods?
I have never had a patient complain of the problem you experienced, and I never cease to be amazed at the number of entertainment personalities you look over operated. The objective of facial rejuvenation surgery is to look better, not so different that you look like someone else. The one exception to this is a rhinoplasty, where the usual objective is to look different. (Despite that fact, often people do not really notice that you had your nose done.) Facial fillers, botulinum toxin and skin rejuvenation procedures should not make you look different but are not a substitute for good surgery when surgery is needed.
My best friend from high school and I both want to have a rhinoplasty procedure done together in time for our 10 year reunion. Do you recommend friends going under the knife at the same time like this or should one of us go first so that we can better advise the other on the process?
