Botox® Treatments

Dr. Bentkover has used Botox® successfully for facial rejuvenation since 1995.

Botox® is great maintenance.

What is Botox® and How Does it Work?

Botox® is part of a class of drugs called neuromodulators. Specifically, these drugs use botulinum toxin type A to decrease muscle activity in the area of injection. Botox® treats what are called “dynamic” wrinkles and creases, muscles and creases caused by the movement of the muscles in the area. The botulinum toxin blocks the release of the chemical that nerves use to stimulate muscles, acetyl choline. Botox® is injected in such a way as to block the signal from specific nerves to specific muscles. In this way it can precisely block the action of a muscle or muscles that cause particular dynamic wrinkles or creases in your face. It is important for a physician to know the location of these muscles and understand the interplay between various facial muscles in order for you to have a successful treatment. It is often an issue of balancing the actions of one set of muscles against another. The effects of a typical treatment last about 3-4 months.

Areas treated include:

  • The crow’s feet around your eyes
  • The worry lines or scowl lines between your brows (the “11’s” in the area called your glabella).If these lines are very deep, you may also want a facial filler in this area.
  • The Forehead for a “Botox® Browlift” to elevate your brow.
  • The horizontal lines of your forehead.
  • The chin creases caused by over-active muscles in that area.
  • The muscles under your chin.

Dr. Bentkover also often uses Botox® in conjunction with fillers like Radiesse®, Belotero Balance®, Juvederm®, Perlane®, or Restylane®, and with some of our lasers. Deep creases or wrinkles in the lower face or around the mouth may be better treated with a facial filler, laser resurfacing or plasma resurfacing.

Smoother Skin With Little Or No Loss Of Movement

Botox® is most commonly injected into facial muscles to smooth wrinkles caused by those muscles. There are also often very tiny muscle fibers in or just below the skin. If a very small amount of very dilute Botox® is carefully injected just into the skin, it can affect just these tiny fibers and smooth the skin without reducing movement significantly. We commonly do this in the forehead and over the cheekbones just beyond the crow’s feet with consistent results. And, because the product is very dilute, the cost to you can be less. An effective Botox® treatment depends on a number of factors:

  • The number of units delivered in each injection site. If too few units are used, the result may be less effective and shorter acting. We will not skimp on the amount of product injected. Dr. Bentkover usually uses 25 units of for the glabella, not just the 20 units used by many injectors. He thinks this gives a better result. In the crow’s feet he uses 10-15 units on each side depending on your needs; under the tail of the brow he uses 2.5-5 units; for the forehead he usually uses 10-15 units.
  • The injector’s knowledge of the function, location and interplay between the muscle groups being injected in your face. A Facial Plastic Surgeon works with these muscles routinely every day.
  • The injector’s overall knowledge of facial aesthetics–what looks good and what does not look good.
  • A customized treatment for you. A commitment to using whatever amount of Botox® takes to achieve your individual aesthetic goals.
  • The secret to maintaining the look you want is to stay on a regular schedule.

As a Board Certified Facial Plastic Surgeon, Dr. Bentkover applies his knowledge of facial aesthetics and the interplay between the muscle groups being injected to try to give you as balanced and natural appearing a treatment as possible. The results of a Botox® treatment should be obvious to you but subtle to others. Dr. Bentkover uses as high an effective a dose of the product as needed to customize your treatment. If you need a higher or lower dose in a particular location, he will adjust the dose to try to achieve what you want. How your muscles react to botulinum toxin is very individual. It can vary from person to person and from treatment to treatment. This is why we offer all three products, Botox®, Dysport® and Xeomin®.

YOUR PROCEDURE

What are the Risks of the Treatment?

  • A treatment with Botox® is very safe.
  • The most common side effect is a transient headache. Some patients can get flu-like symptoms for a few days.
  • The most common noticeable side effect is called ptosis, or drooping of the eyelid when injected between the brows. We think this is from rubbing the area injected in the first 24 hours.
  • All complications are rare complications, especially any issues with the eyelids.
  • The FDA requires what is called a Black Box warning for all types of botulinum toxin injections, Botox®, Dysport® and Xeomin®. If you have a neuromuscular condition with weakness of the muscles or difficulty swallowing, you are not be a candidate for these treatments.
  • Allergic reactions are very rare.
  • We do not treat pregnant or nursing women; these products are not cleared for them.
  • Botulinum toxin can inhibit swallowing or breathing when injected in doses far beyond above the dose used for cosmetic treatments. Overdoses should be unheard of in the doses used on the face in adults.
  • Some patients can develop an immunity to Botox®. If this happens, Dysport® or Xeomin® may work.

If you take aspirin, ibuprofen, warfairn or any other blood thinners on a daily basis, you should stop this type of medication 10 days before the injections, if it is safe for you to do so. If it is not safe for you to stop such medications, your risk of bruising, while small, will be higher with the treatment.

The Procedure

Injections are usually performed without topical anesthesia, but we have it available if you want it. We can also apply ice before the injection to numb the area. The discomfort is minimal, and most patients require no topical anesthetic or ice. A small sting is felt with each injection. The effects of the treatment are not permanent and should be repeated every 3-4 months to maintain the correction.

FAQs

What are Botox®, Xeomin® and Dysport®?

They the purified form of a toxin produced by a bacteria called clostridium botulinum. There are many misconceptions about purified botulinum toxin. It is not a poison. Many other medications are made from purified toxins and used safely.

Are Botox®, Xeomin® and Dysport® safe?

YES, VERY SAFE. The active agent in these products, Botulinum ToxinType A, has been used for over 25 years to treat conditions like facial muscle spasms. In children, it is used to treat strabismus (“lazy eye”) and to loosen muscles for children who need physical therapy for cerebral palsy. Like any other drug, it must be used in doses appropriate for the age and size of the patient. Serious complications have occurred when it is used in doses that far exceed recommended doses. Dr. Bentkover is very careful with his dosing.

Does everybody get the same dose?

While we know from experience approximately the dose that you will need in a given area, some patients require more and some less. We will see you usually 3 weeks after your first treatment to evaluate the result. At that time, we will “top off” the area at no additional charge, if you need more. At subsequent treatments, we will use the higher dose, and the fee will reflect the additional cost of the medication. Rarely, patients require less than the initial dose. If you require less for subsequent treatments, the fee will also be adjusted.

Are the results always the same?

While most people usually get about the same result with every treatment, the results can vary somewhat from treatment to treatment. Also, although it is very unusual, some people seem to develop immunity to Botox® over time. Often this can be reversed by skipping a couple of cycles (4-6 months). If this dose not work, Xeomin® or Dysport® will often work in patients immune to Botox®. Conversely, some of our long term patients report that their treatments tend to last longer after a number of years of treatment.
